WAIVER/RELEASE AGREEMENT: By completing this form, I acknowledge and fully understand that I will be engaging in activities that involve risk of serious injury which might result not only from my action, inaction or negligence, but the action, inaction or negligence of others, the rules of play or the conditions of the premises or of any equipment used and that there may be other risks not known to me or not reasonably foreseeable at this time; I release, waive and discharge any and all claims of damages for death, personal injury or property damage which I may have or which may hereafter accrue to myself as a result of my participation in the Shootout or otherwise and which may be asserted by me on my behalf against the MAAC, the host institution, the NCAA or any of their respective sponsors, suppliers, affiliates, administrators, officers, agents, coaches, directors, employees, other participants in the Shootout, advertisers or owners or lessees of the premises used to conduct the Shootout. I accept the judges' decisions in determining the results of the Shootout; I grant permission for the use of my name and likeness of me for promotional or publicity purposes without additional compensation. This waiver and release applies to all levels of competition for the Shootout.
